Coverage above those amounts, late enrollments, or future increases may require Evidence of Insurability (EOI) and approval by the insurance carrier.

 

Voluntary Life Insurance Overview

Voluntary Life Insurance provides a benefit to a designated beneficiary in the event of a covered death, subject to the terms of the policy.

Coverage may be elected for:

  • The team member

  • An eligible spouse

  • Eligible dependent children

Guaranteed Issue vs. Medical Certification (EOI)

Guaranteed Issue (When Available)
