Only benefit changes that are directly related to the qualifying life event are permitted.
Important Deadlines

You must report the qualifying life event within 31 days of the event date.

Requests submitted after the 31-day window cannot be accepted under plan and IRS rules.

Supporting documentation (such as a marriage certificate, birth certificate, or proof of coverage loss) is required.
How to Request a Benefit Change Due to a Qualifying Life Event
Step 1: Submit a Life Event Declaration

Log in to Dayforce under your Employee profile.

Select the Menu in the top left.

Choose Benefits from the menu bar.

Select the Forms tab at the top of the Benefits page.

Complete and submit a Life Event Declaration.
Step 2: Complete Benefits Enrollment (After HR Review)
Once your life event is reviewed and approved by HR:

Select the Menu in the top left.

Choose Benefits from the menu bar.

Available enrollment options will appear under the Enrollments section at the top of the page.

Complete your benefit elections and submit.
Coverage changes generally become effective on the date of the qualifying life event or as required by the benefit plan.
