A qualifying life event is a change in life circumstances that allows individuals to change their health insurance benefit elections outside of their initial enrollment or open enrollment period.
Qualifying life events include, but are not limited to:
- Marriage, divorce, or legal separation.
- Birth or adoption of an eligible child.
- Death of your spouse or covered child.
- Change in your spouse’s work status that affects his or her benefits.
- Change in your child’s eligibility for benefits.
- Qualified Medical Child Support Order.
To request a benefits change due to a qualifying life event, please complete the following steps:
- Complete a Life Event Declaration in Dayforce
- Log in to Dayforce under your Employee profile
- Select the MENU in the top left.
- Select BENEFITS from the menu bar.
- Select the FORMS tab on the top of the BENEFITS page.
- Complete & submit a Life Event Declaration.
- Contact Human Resources within 31 days of the qualifying life event. Change requests submitted after 31 days cannot be accepted. You will be required to provide proof of the event, such as a marriage license or birth certificate.
- Once approved by HR, the Dayforce Benefits Enrollment will be available for you to complete
- Log in to Dayforce under your Employee profile
- Select the MENU in the top left.
- Select BENEFITS from the menu bar.
- Available enrollment options will appear at the top of the page under the ENROLLMENTS section.
